UvoWell is a trade name (DBA) of UVO, LLC and provides access to telehealth services through independent licensed healthcare providers and clinical partners, including OpenLoop-supported provider organizations where applicable.

Your Rights
You have the right to:
- Obtain a copy of your medical record
- Request corrections to your medical record
- Request confidential communications
- Request certain restrictions on disclosures
- Receive an accounting of disclosures where required by law
- Obtain a copy of this notice
- Designate an authorized representative
- File a complaint if you believe your privacy rights have been violated
Our Uses and Disclosures
We may use and disclose your PHI for treatment, payment, and healthcare operations, including coordinating your care with licensed healthcare providers, pharmacies, laboratories, and other service providers involved in your treatment.
We may also use PHI as otherwise permitted or required by HIPAA and applicable law, including:
- Public health activities
- Research where authorized
- Legal compliance
- Law enforcement requests
- Workers' compensation
- Organ donation
- Judicial proceedings
Your Choices
For certain disclosures, such as communications with family members involved in your care or marketing communications, you may have choices or rights to provide or withhold authorization as required by law.
We will obtain your written authorization when required before using or disclosing PHI for purposes that require authorization.
Healthcare Services
Medical services are provided by independent licensed healthcare providers. UvoWell does not practice medicine or pharmacy. Providers independently determine whether treatment, laboratory testing, or prescriptions are appropriate. Prescriptions are fulfilled by licensed pharmacies.
Security
We maintain administrative, technical, and physical safeguards designed to protect PHI in accordance with HIPAA and other applicable laws. If a breach affecting unsecured PHI occurs, we will provide notification as required by law.
Changes
We may update this Notice from time to time. Revised versions will be posted on UvoWell.com and become effective as of the posted effective date unless otherwise required by law.
Complaints
If you believe your privacy rights have been violated, you may contact UvoWell at Support@uvowell.com.
You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.
